Dr. Casey Taber on Lady Gaga’s Show Stopping Injury

You may have read in the news that Lady Gaga has called off her tour due to a labral hip tear.  This is a fairly common injury that I see routinely in my practice and is most likely a result of her very active lifestyle.  

The labrum is a piece of cartilage that wraps around the hip socket and helps stabilize the hip joint.  The most common cause of labral tears is Femoral Acetabular Impingement (FAI).  Which means the thigh bone (Femur) pinches on the hip socket (Acetabulum) and causes trauma to the labrum (which attaches to the outer rim of the Acetabulum), which can result in the labrum tearing.  There are multiple factors that contribute to FAI, but we believe it occurs primarily from a combination of hereditary abnormalities, traumatic events, and activity related injuries.  For instance, there is a much higher incidence of FAI and labral tears in athletes, particulary football players, hockey players, and soccer players than in the general population.

Even if you are not involved in these types of high impact sports, labral hip tears are common injuries found in young, active people.  They can be found in kids as young as 14 and in adults in their elder years.  We tend to see them more in the younger population (14-45), because this is the time period that people tend to be the most active.  Many elderly patients will have labral tears, but will not show symptoms because of their decreased activity level.  

Although some people with labral tears may never require fixation, it is much more common that the tear requires surgery, particularly when seen in a young, active patient.  Surgery is often necessary in order to return to full function and activity/sport.

The initial treatment for labral tears is typically a short period of rest and anti-inflammatory medication.  The usual recommended time period of inactivity is 6-12 weeks.  This can sometimes be supplemented with an intraarticular injection of steroid to calm the acute inflammation down following injury.  If conservative measures fail to provide the desired relief, surgical repair is the treatment of choice.  Labral hip tears are most commonly fixed arthroscopically, although open procedures do exist.  Patients typically require one night in the hospital and 2-4 weeks on crutches.  Full recovery, in particular return to sport, is usually possible within 4-6 months.

Although some of the causes of FAI may be unavoidable and in a patient’s “deck of cards”, prevention is always a key component in any injury.  I typically recommend being very diligent in stretching before participation in any sport or prolonged activity.  Another key component, particularly in females, is developing and maintaining a strong and stable core.  Core strength and gluteal strength are key components in the prevention of all hip injuries, including labral tears.

Dr. Casey Taber Adds Clinic at Stone Oak

Beginning January 2013, Dr. Casey D. Taber will be seeing patients at our North Central / Stone Oak location on Tuesday mornings.  This office is located at 150 E Sonterra Blvd, Ste 300, San Antonio, TX 78258. 

When asked why he decided to add a clinic at Stone Oak, Dr. Taber said, “I wanted to go out to the Stone Oak region for two reasons.  One, I think I can offer the area an expertise in certain areas of orthopaedics that haven’t been available to the people in that area until now.  Secondly, I have a large population of current patients on the north side of town, including north San Antonio, Bulverde, Canyon Lake, Schertz, and New Braunfels, that will appreciate a more accessible office.

In addition to Stone Oak, Dr. Taber will continue to see patients in Downtown and Westover Hills as he has for years.

Local Docs: Kids Are Overdoing Sports

The number of childhood and teenaged sports injuries is skyrocketing to the point where orthopaedic surgeons call it a ‘silent epidemic,’ and they say it is due to the ‘alarming trend’ of young kids participating in sports year round, 1200 WOAI news reports.  

“It is certain something that we are seeing more frequently in the office nowadays,” says Dr. Casey Taber with the San Antonio Orthopaedic Group, the state’s leading practice for research and treatment into bone, muscular, and musculoskeletal injuries. 

 “They are using their bodies more than they probably should.”  

Doctors worry that overuse and traumatic joint and extremity injuries among adolescents are setting them up for a ‘lifetime of chronic pain.’ 

A report released at the annual meeting of the American Academy of Orthopaedic Surgeons this past weekend suggests ‘mandated downtimes’ for young athletes.  

“More adolescents are participating in year-round sports without seasonal breaks,” the report says. “They are playing on multiple teams simultaneously. This increased exposure means there will continue to be growing numbers of significant musculoskeletal injuries, both traumatic and chronic overuse.”  

Dr. Taber sees it in his clinic, and he doesn’t like what he sees. 

 “You are starting to see some injuries nowadays that we didn’t see years ago,” Dr. Taber says.  

He says more and more, he encounters adolescents who are participating in year-round sports, and occasionally in several sports at once. 

“They are using their bodies more than they probably should,” he said.  “The only way to prevent that is to back off to some degree.” 

He says parents need to be aware that even young, strong, and physically fit young people can suffer chronic injuries by overuse. 

The report says ‘everyone wants to get to the top,’ but adds, “we have to look at this and say, are we pushing kids too hard?  We cannot wait for kids to reach the college level to modify their training, because by that time, it could be too late.’ 

Orthopaedic surgeons point out that college and pro sports governing bodies regulate how much practice, when practice can begin, and how many sports people can participate in, but frequently, there is none of that regulation at the grade school and high school level, where the potential for lifelong damage is the greatest.
