Dr. Christian Balldin on Rams QB Sam Bradford’s Torn ACL

The quarterback for the St. Louis Rams, Sam Bradford, tore his Anterior Cruciate Ligament (ACL) this past weekend, ending his current season.  Unfortunately, ACL tears are a common injury in sports like football that have significant pivoting and change of direction as part of the game.  They can occur as part of a traumatic injury (where the knee is put in an unnatural position and the ligament tears due to excessive force applied to the knee) or as part of a non-contact pivoting injury, which is actually the most common mechanism of injury.  

Sam Bradford’s injury means his season is over because his injured knee will not be able to withstand the stress upon it during attempts at change of direction. The ACL is the main stabilizer in keeping the lower part of the leg (the tibia) from not excessively moving forward and rotating from the thigh bone (the femur). 

Often, ACL injuries occur at the same time as other injuries – such as other ligament tears, meniscal tears and cartilage injuries.  If other injuries are sustained, the surgery required will be more complex.  Meniscus tears, if possible, should be treated with repair rather than simply removing the damaged tissue to allow the meniscus to perform its job which is to act as a cushioning between the shin bone (the tibia) and the thigh bone (the femur).  This allows the cartilage to see less stress and have a lower chance of injury over time.  

There are many choices of graft alternatives for reconstructing an ACL.  One can decide to use a patient’s own hamstring or patellar tendon.  There are pros and cons with each option.  The gold standard in the NFL is the use of the patellar tendon, but we treat each patient on a case by case basis and determine the proper graft choice based upon the circumstances.  

Rehabilitation following surgical ACL reconstruction is very important and will last at least 6 months for most patients.  I believe in therapy that involves great communication between the physician, patient and the therapist.  Each patient is different and protocols, although great, sometimes requires tweaking to allow for the best results.  

Assuming Sam Bradford’s injury is an isolated ACL tear, with the proper treatment he has a good chance of being fully recovered and ready for the start of next season.

Dr. Casey Taber on Orthopaedic Opportunities for Ambulatory Surgery Centers

Becker’s ASC Review
recently interviewed TSAOG’s Dr. Casey D. Taber, along with 2 other orthopaedic surgeons, to discuss some of the biggest opportunities for the field of orthopedics in ambulatory surgery centers (ASCs) in the coming years.

According to Dr. Taber, one of the biggest areas of opportunities is transitioning many types of hip arthroscopy procedures into the inpatient setting of an ASC.  Dr. Taber says that performing these relatively new procedures in an ASC will be less costly, not to mention “easier on the patients and easier on the physicians.”

TSAOG Congratulates River City Run

TSAOG would like to congratulate River City Run, a different kind of running activity, on its one year anniversary this weekend!  River City Run is extra special to us because it is owned and operated by TSAOG Surgery Center R.N. and small business entrepreneur Tina Stolhandske and her husband Tommy.

River City Run is a 5k running tour that gives participants the chance to see the sights of downtown San Antonio while getting some exercise!  Tour groups meet in front of the Alamo every Saturday and Sunday at 8am and take off for an educational 5k with 15, 2-3 minute stops along the way to see the sights and learn the history of downtown San Antonio.  Cost to participate is $30/person, which includes a t-shirt and water bottle.  Note: River City Run does request pre-registration for tours on their website. Walking tours, private tours, and group rates are also available.

Only one year from its start, River City Run has made quite an impression the Alamo City.  Over 800 runners have participated in its events and it has claimed the #1 spot on Trip Advisor’s List of Activities in San Antonio!

Dr. Alexander Rowland on the FAST Technique for Tennis Elbow

Tennis elbow (lateral epicondylitis) is the most common elbow ailment for which patients seek medical attention.  It is more commonly seen in patients with repetitive overuse injuries but can also be seen after a single traumatic event.  Although tennis elbow was initially thought to be an inflammatory condition, it is now known to be a degenerative process with incomplete tendon healing.

Patients suffering from tennis elbow generally complain of pain on the lateral (or outer) aspect of the elbow. Symptoms are aggravated with gripping and lifting and alleviated with rest. The good news is that the majority of patients are symptom free at 1 year regardless of the type of treatment they receive. 

Nonsurgical management of tennis elbow generally involves a combination of rest, anti-inflammatory drugs, physical therapy, braces, and steroid injections. Comparative studies have shown no single nonsurgical treatment to be superior.  

Operative management has generally been reserved for patients whose symptoms have not improved with conservative treatment. The two most commonly performed surgeries involve tendon debridement (cleaning up and removing scar tissue) either with an open incision or arthroscopically. The advantage of arthroscopic treatment over open surgery is that patients are able to return to normal activities sooner and have no scars. The disadvantage of either of these surgical techniques is that they require general anesthetic and often have a lengthy recovery time which can take up to 6 months.

Another treatment option for tennis elbow is the FAST technique, a relatively new technique developed in collaboration with Dr. Bernard Morrey at the Mayo Clinic for the treatment of chronic tendon injuries. It involves percutaneous removal of abnormal tissue under local anesthetic and ultrasound guidance. The procedure is performed after ultrasound evaluation to localize the abnormal tendon.  A local anesthetic is then given to numb the skin and tissue. Through a small (3mm) incision a probe is placed to cut and remove abnormal tissue and help restore normal tendon function. A small dressing is placed over the incision to allow immediate motion without immobilization.

The entire treatment takes about 15 minutes. Patients are allowed early use of the arm for daily activities the following day, but should avoid heavy lifting for 6 to 8 weeks.  Complete recovery is expected in 8 to 12 weeks. I have decided to incorporate this treatment into my practice mainly because there is little down time, minimal pain, and the recovery time is half or less of standard operative techniques.  The goal is to get patients back to work and a more normal lifestyle in the quickest way possible.
