Initial Evaluation

Comprehensive health review of your medical history, current health concerns, medication review, and physical examination. Based on the evaluation, the physician will create a personalized care plan that addresses your health needs and goals. This service is a single visit package, and does not include follow up visits, fracture care, surgical treatments, MRI, physical/ occupational therapy or orthopedic bracing and any additional testing. Follow up visits and other services can be purchased separately if needed.

If your physician orders an X ray during the visit you will not be charged for the additional service.
