TSAOG Orthopaedics & Spine partners with HealthMark Group to efficiently manage and process all requests for medical and billing records.
All TSAOG requests are securely processed through HealthMark to provide a streamlined, timely, and user-friendly experience. Users will be redirected to our HealthMark partner platform to submit their request and complete the process.

If you would like to request medical, billing, and/or imaging, please use the link to be redirected to our partner, HealthMark, to submit your request.
Note: Mical and billing records mailed out over 600 pages may incur a shipping fee as well as documents over 600 pages. Please contact HealthMark at 1-800-659-4035 for further information

FMLA or Disability Forms
To have FMLA or disability documents completed and signed, use the “Upload FMLA or Disability Documents” link. Based on your provider’s preference, you will either be redirected to HealthMark or to the patient portal to begin the process.
Note: You will receive an invoice from HealthMark for the completion of FMLA or Disability documents. These documents will only be released once payment is received. Upon receipt of payment, you will receive an email from HealthMark notifying you when your FMLA or Disability documents are available for download.

Note: To make a payment you will need to add the Invoice Number and Access Code into the HealthMark system.
Medical & Billing Records and FMLA/Disability Paperwork – Will only be released once payment is received by HealthMark.

Third-Party Requestors
Third-party requestors, such as attorneys seeking medical and/or billing records, are advised to upload their HIPAA-compliant authorization form using the link provided below.
For third-party requests, records may not be released if the required fee is unpaid or if valid HIPAA authorization and additional documentation are missing. If authorizations are incorrect or missing, you will be contacted via the email provided.
